Requesting your records.

Cochran Memorial Hospital and Morton Rural Health Clinic maintain medical records in strict confidence. Records are released only with proper authorization.

Important: Cochran Memorial Hospital and Morton Rural Health Clinic are housed in the same building, but they are two separate entities. Inpatient and Emergency Department medical records are kept by Cochran Memorial Hospital. Clinic charts are kept by Morton Rural Health Clinic. If you are requesting records from both Cochran Memorial Hospital and Morton Rural Health Clinic, you will be required to complete two separate forms. Contact information for each medical records department is provided below.
Requesting Medical Records

Cochran Memorial Hospital and Morton Rural Health Clinic maintain medical records in strict confidence and records are only released with proper authorization. A patient, parents of a minor child, or caregivers acting on behalf of a patient, such as Medical Power of Attorney, may inspect or request copies of medical records, or have copies of medical records sent to another provider or facility.

Below we have provided important information on accessing and/or requesting your medical records, ways to access and/or request your records, and the required forms and contact information.

All records are retained for the period of time specified by the State of Texas. Adult patient records are retained for ten (10) years from date of service. Minor patient records are kept to the age of twenty-one (21) or ten (10) years from date of service, whichever time period is greater.

Important: We are unable to fax or electronically send records to individual patients. Patients must pick their medical records up in person or request that they be sent by certified mail.

The fastest way to access many of your medical records is to use the My Care Corner Patient Portal. The Patient Portal allows you to access your medical records on demand any time you need them.

Hospital and Emergency Department Medical Records

Hospital and Emergency Department records include:

  • Inpatient stays
  • Observation stays
  • Custodial stays
  • Hospice/Respite care
  • Emergency Room visits

To request records, you will need to print and complete the appropriate form below.

Records can be picked up in person or sent by certified mail. We will not send records by email or fax to individual patients.
You must bring a valid form of photo identification with you when picking up medical records. Acceptable forms of identification include:
  • Driver’s License
  • State Identification Card
  • Passport
We cannot accept copies or photographs of identification documents. All forms of identification must be in-date to be considered valid.
Morton Rural Health Clinic Medical Records

Morton Rural Health Clinic records include:

  • Office/Clinic visits
  • Laboratory
  • Radiology
  • Outpatient visits

To request records, you will need to print and complete the appropriate form below.

Records can be picked up in person. We will not send records by email or fax to individual patients.
